چکیده

SINCE their introduction by Lopez and coworkers,' paired electrical stimulation and coupled pacing of the heart have been investigated intensively and have been shown to be useful in slowing the effective mechanical rate of the heart or in causing sustained potentiation of cardiac contraction or for both. A recent symposium on this subject provides an excellent review.2 The techniques, in essence, consist of electrically stimulating the heart shortly after the termination of the effective absolute refractory period of each previous spontaneously or electrically driven beat. These stimuli cause an artificial bigeminal rhythm, the second member of each pair being mechanically ineffective. In paired stimulation (PS) both the mechanically effective beat and the coupled extrasystole are pacemaker induced, while in coupled pacing (CP) a spontaneous depolarization is permitted to occur and only the extrasystole, triggered by the preceding R wave, is artificially induced.3-5 Each extrasystole produced by either of these methods possesses a refractory period of its own and thus causes a doubling of the myocardial refractory period to other stimuli. This lengthened refractory period can be utilized to control tachycardias.' 6 In addition, the introduction of such extrasystoles causes postextrasystolic potentiation (PESP) of the next mechanically effective beat. By repetitive coupling of extrasys-

Effects Of Fast Pathway Ablation On The Concealment Zone And Electrophysiological Behavior Of AV-Node During Atrial Fibrillation.

Objectives: Dual pathways have a determinant role in the occurrence of atrio nodal tachyarrhythmia (AVNRT). The aim of present study is to determine the role of slow pathway (SP) in the concealment zone and protective role of AV node during atrial fibrillation (AF). Material &Methods: In 7 isolated nodal rabbit preparation zone of concealment and concealed conduction is quantified by Specif...

Intractable paroxysmal tachycardia caused by a concealed retrogradely conducting Kent bundle. Demonstration by epicardial mapping and cure of tachycardias by surgical interruption of the His bundle.

Electrophysiological and epicardial mapping studies are described in a patient without pre-excitation who had intractable recurrent paroxysmal supraventricular tachycardia. Electrophysiological studies revealed fixed VA conduction times during both rapid ventricular pacing and coupled ventricular stimulation. Catheter mapping of atrial activation during retrograde conduction and during induced ...

Concealed paroxysmal atrioventricular block: diffuse congenital atrioventricular conduction system disorder with nonpropagated His bundle depolarizations.

A 2 1/2-year-old girl with bradycardia and left bundle branch block at birth began to experience "night cries" when deeply asleep. Electrophysiological study demonstrated congenital diffuse atrioventricular conduction disease with concealed paroxysmal atrioventricular block, nonpropagated His bundle depolarizations, severe sinus node abnormality, and a low atrioventricular junctional escape rhy...

Dynamics of conduction blocks in a model of paced cardiac tissue.

We study numerically the dynamics of conduction blocks using a detailed electrophysiological model. We find that this dynamics depends critically on the size of the paced region. Small pacing regions lead to stationary conduction blocks while larger pacing regions can lead to conduction blocks that travel periodically towards the pacing region. We show that this size-dependence dynamics can lea...
